Community Wellbeing Officer (Pembrokeshire)

Salary: £19,544 - £22,462 

Location: County Hall, Pembroke

Delta Wellbeing provides a multi-function call/contact handling service 24x7 across south-west Wales available to people who use social alarms, it also deals with out-of-hours emergency calls for council services and is an initial point of contact for social care to Adults in Carmarthenshire.

We have a number of vacancies available for innovative, flexible, motivated individuals who will excel in working as a key part of our team to deliver an exciting new model of pro-active Technology Enabled Care (TEC) services as part of the Welsh Government’s Transformation Fund project across the West Wales Partnership Board region.

Employed by Delta Wellbeing the posts will be hosted in Pembrokeshire by one of three partner organisations; Hywel Dda University Health Board (HDUHB), within the Pembrokeshire Community Directorate, Pembrokeshire Association of Voluntary Services (PAVS), within the Community Connector team; and the British Red Cross, within the PIVOT Caseworker team.

In line with the new model of service you will be required to carry out Pro-active Wellbeing assessments for service users in their own home or in community settings, with follow up support and review as appropriate. You will be a key member of the Pembrokeshire Team providing Information, Advice and Assistance regarding community services for members of the public, outside agencies, health colleagues and other stakeholders requiring support.

Awareness of the issues associated with working with elderly or vulnerable people would be desirable, along with information about social care, and/or other services provided by the local authority, health and the 3rd sector.

Candidates should be able to communicate effectively with a wide range of people and provide excellent customer service over the phone and in person. 

You will receive a comprehensive induction training programme when you join us and on-going training to support your development.

Conversational Welsh is desirable to accomplish this post. Support can be provided on appointment to reach this level.

We currently have a number of vacancies available. We are looking for flexible, motivated individuals who will excel in working as a key part of our team to deliver an exciting new model of pro-active Technology Enabled Care (TEC) services as part of the Welsh Government’s Transformation Fund project across the West Wales Partnership Board region.

You will form part of a team providing Information, Advice and Assistance for all social care and health services for members of the public, outside agencies, health colleagues and other stakeholders requiring support. In line with the new model of service you will be required to carry out Wellbeing assessments for service users in their own home, with follow up support and review as appropriate.

Awareness of the issues associated with working with elderly or vulnerable people would be desirable, along with information about social care, and/or other services provided by the local authority.

Candidates should also have a good understanding of computers, be able to communicate effectively with a wide range of people and provide excellent customer service over the phone and in person.

You will need to be qualified to NVQ4 (or equivalent) in Information, Advice & Assistance or similar Health & Social Care qualification.

Technology Enabled Care Officer

Salary: £21,589 - £25,295

Delta Wellbeing is looking to recruit a new Technology Officer into its team in Carmarthenshire. This post will be key in the delivery of the service and its ongoing growth in the County. Candidates should have an understanding of the Telecare service and be practical, as small power tools are used to install some Telecare items.

Empathy with those who are suffering from illnesses and the ability to talk to people at all levels is essential for this post as it is a vital skill in the prescription of Telecare equipment.

Some weekend working is included in this role, and a current driving license is required.

A basic knowledge of Welsh is required to accomplish this post. Support can be provided on appointment to reach this level.
